If you don't use anything to sleep with, it can flatten out the neck. In the neck we often will have a 40 - 60 degree curve. With no neck support will lose the normal curvature.
Remembering our anatomy, the brain sits on top, sends messages down the spinal cord, out the nerves, to all parts of the body. If we have interference up in the neck, not only might we get neck pain, but anything below that point can be disrupted too.
So to answer the question, it isn't good to sleep without a pillow. Using the Arc4life Traction Pillow, is a great pillow to start with that gives a very gentle support. There is a support side as well as a traction V Side which gently stretches your cervical spine.
