It's possible, because it may have lost support, but you didn't notice. If you got used to it as it, over the months and years, you may not have noticed. it's not all the time where you wake up with neck pain or arm pain and know that your pillow has reached the end of the rope.
Here's what I recommend, lay the pillow on the bed and simply look at it. Does the neck roll look flat? Does it only come a couple inches from the surface of the bed? For comparison, the traction pillow is about 4 inches off the bed. When you lay on it it will compress an inch or two, but there will always be enough support. So if your pillow only starts 2-3 inches above the surface it may squish down to nothing?
Next, push on that neck roll. 1) does it easily compress all the way to the bed? 2) Does it quickly bounce back up when you take your hand away? If not, the pillow may be on the way out...
Analyze that pillow and decide if it's time for a new one. Also, if it has been many years, there's a good chance it needs to be replaced.
